Scholarships:

BIAV members: Please fill out this form and return it to the BIAV office by May 8, 2020. If applying for more than one child, fill out a new form for each of your children. Note: The form must be printed, signed and sent to the BIAV office. If you need any assistance completing the form, printing it or so on, please call Levi Perl at (913) 735-9612 for assistance and/or to schedule a time that you and the child(ren) you are applying for can fill and sign it.
